Limited Liability Waiver & Photo Release form

Waiver Agreement

I am of legal age, sound mind and voluntarily agree to all terms and conditions below:

​

I am receiving a sound bath session conducted by Rising with V LLC. I understand that Rising with V LLC provides a simple, gentle, energy based sound & vibrational technique used for stress reduction and deep relaxation. These sessions include but are not limited to; listening to Crystal bowls, Gongs, Chimes, Ocean Drum and other healing instruments around the body, vibrations on the body, spiritual arts, laying on a massage table or floor, aromatherapy, Reiki, and/or other energy modalities or techniques.

I consent I am in good physical health and of sound mind and do not suffer from any physical or mental conditions which would hinder my participation in this session.

I understand my session will not take the place of professional medical or health care which is 100% my responsibility and is recommended by Rising with V LLC I see a licensed physician or healthcare professional for any physical or psychological ailments or concerns that I may have. 

I understand that Rising with V LLC does not diagnose conditions, make recommendations, prescribe, perform medical treatments, nor interfere with professional medical treatments of a licensed healthcare professional.

Any medical or mental health advice given is ultimately accepted by my own free will and I release any and all liabilities including but not limited to legal, financial and personal liability to Rising with V LLC and Veronica Pereira.

I understand and agree that Rising with V LLC is not claiming the ability to heal my body or mind.

My purchase of services and participation in all activities provided by Rising with V LLC is confirmation I am fully responsible for any and all risks, consequential damages, injury, incidental damages, personal damages or any other damages known or unknown without limitation.

We may make changes in our waiver agreement and Terms & Conditions (on our website) from time to time. It is your sole responsibility and encourage you to check back at any time to see our current policies.

By signing this release Rising with V LLC and Veronica Pereira. I herby waive and release all liability, past, present and future relating to Rising with V LLC.

I, hereby grant and authorize Rising with V LLC the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me by Rising with V LLC to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. 

This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing.

I understand and agree that these materials shall become the property of Rising with V and will not be returned.

I hereby hold harmless, and release Rising With V LLC from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate.

I have received this policy statement and have read and agreed to all of the policies therein.
